Beating Unexplained Infertility

Even IVF treatment can fail at times, and the reasons can be difficult to identify.

Mrs. Y and her husband had been visiting Metro IVF since 2022 and finally decided on IVF treatment last year. Screening showed no significant fertility issues, classifying their condition as ‘unexplained infertility.’

Their IVF treatment resulted in six beautiful day-five embryos (blastocysts), all graded A+ to A- (4AA to 6AB).

However, their first FET last year was unsuccessful, with Beta HCG levels rising only to 7 before dropping. For the couple’s second FET, additional measures were taken but their Beta HCG level peaked at 12 before dropping again. It was perplexing why such high-quality embryos, an easy transfer, and good hormone levels did not result in a pregnancy. They were then advised to send the remaining two embryos for genetic testing (PGT-A). Genetic issues in embryos, despite appearing excellent in grade, can be a cause of implantation failure.

After discussion, they decided to try a third time using the last two embryos, with plans of undergoing a new IVF cycle with PGT-A if they failed again. Their third FET attempt included intralipid medication to further reduce excessive immune response that could prevent embryo implantation.

As a final effort, low molecular weight heparin blood thinners were included in their daily injections too. The joy of the couple was immense when this time, their Beta HCG levels reached 717, indicating a successful pregnancy!

This couple’s journey highlights that not all IVF attempts are successful on the first try. Each step needs careful review and adjustment by a medical expert.
